Guitar Hero: Aerosmith to kolejna gra z bestsellerowej serii, łączącej muzykę z elementami zręcznościowymi. Dokładnie tak jak sugeruje nazwa, aplikacja koncentruje się przede wszystkim na twórczości rockowego zespołu Aerosmith, wywodzącego się z Bostonu. Przy produkcji brali udział także sami muzycy tej popularnej, amerykańskiej grupy.

W trakcie zabawy w Guitar Hero: Aerosmith gracze mogą pokierować losami Joe Perry’ego. Reszta ekipy stanowi jedynie tło dla naszych indywidualnych popisów. Celem zabawy jest wypromowanie zespołu od mało komu znanej kapeli, do uwielbianych na całym świecie legend rocka.

Ogólny mechanizm rozgrywki nie zmienił się w stosunku do Guitar Hero III: Legends of Rock. Zabawa polega więc na naciskaniu odpowiednich przycisków gitarowego kontrolera, zgodnie z rytmem aktualnego utworu. Im lepiej nam to zadanie wychodzi, tym wyższe uzyskujemy wyniki punktowe. Każdy popełniony błąd wpływa zaś ujemnie na poziom zadowolenia widowni.

Wśród tła dźwiękowego dominują utwory Aerosmith. Ponadto jednak gra zawiera kawałki innych wykonawców. Oto pełna lista utworów:
